Progesterone deficiency. Progesterone is produced primarily by the corpus luteum after ovulation. In perimenopausal women, ovulation becomes less consistent - and when ovulation does not occur, progesterone production in that cycle is absent or minimal. The result is a cycle dominated by estrogen without adequate progesterone opposition. This anovulatory pattern begins years before menopause and is one of the earliest and most clinically significant hormonal changes of perimenopause.
Excess peripheral aromatization. Adipose tissue contains the enzyme aromatase, which converts androgens to estrogens. In higher-BMI patients, greater aromatase activity produces more peripheral estrogen - contributing to a state of relative estrogen excess that is directly proportional to body fat percentage. This mechanism is particularly relevant in the metabolic health and weight management patient population.
Impaired estrogen metabolism and clearance. Estrogen is metabolized in the liver and excreted primarily through the gastrointestinal tract. The symptom profile of estrogen dominance is distinctive and, once recognized, guides both diagnosis and treatment direction. Providers who are familiar with this symptom cluster can often identify estrogen dominance as a primary or contributing diagnosis before labs are even ordered.
The most common symptoms include weight gain concentrated in the hips, thighs, and lower abdomen. Breast tenderness, fullness, and fibrocystic changes are also common, as is bloating and water retention that worsens in the week before menstruation. Heavy, prolonged, or irregular menstrual periods are frequently present. Premenstrual mood changes including irritability, anxiety, and emotional lability are characteristic. Fatigue that is disproportionate to sleep duration and quality, decreased libido, and difficulty losing weight despite adequate effort are also consistent features of the clinical picture.
Many of these patients have been told their labs are normal and their symptoms are attributable to stress, depression, or aging. They are often on antidepressants or anxiolytics prescribed for mood symptoms that are actually hormonally driven. Identifying the hormonal component does not invalidate the mental health treatment - but it does identify a treatable underlying cause that the mental health treatment alone will not address.
LAB PATTERNS THAT SUPPORT THE DIAGNOSIS
While estrogen dominance is primarily a clinical diagnosis supported by symptom pattern and history, lab findings can confirm and quantify the imbalance.
Estradiol and progesterone. The estradiol-to-progesterone ratio is the most informative calculation. This level of detail is not necessary in all cases but is valuable in complex presentations.
TREATMENT STRATEGIES
Treatment of estrogen dominance addresses the underlying drivers rather than simply suppressing estrogen levels. The most effective approach is multimodal - combining progesterone rebalancing, estrogen metabolism support, gut health optimization, and lifestyle modification.
Progesterone rebalancing. For patients with documented or clinically apparent progesterone deficiency, bioidentical progesterone supplementation is the cornerstone of treatment. Oral micronized progesterone at physiologic doses - typically 100 to 200 mg nightly - addresses the progesterone deficiency that allows estrogen to dominate. In premenopausal women with regular cycles, progesterone is typically supplemented in the luteal phase only, days 14 through 28, to mimic the natural progesterone pattern. In perimenopausal and postmenopausal women, continuous dosing is more common.
DIM supplementation. Diindolylmethane is a compound derived from cruciferous vegetables that supports healthy estrogen metabolism by promoting the conversion of estrogen toward the 2-hydroxyestrone pathway - a less estrogenically active and potentially protective metabolite - and away from the 16-alpha-hydroxyestrone pathway, which is more estrogenically potent.
